We know patients have questions about all things related to their care. We’ve answered those most frequently asked below.

In order to obtain a copy of your medical records, please call our office at and ask for the medical records department.
Call our office at or and let the receptionist know you’d like to schedule an appointment. They will connect you with the appropriate scheduler to help you with your appointment.

If you have a life-threatening problem, you should go straight to an ER.

For urgent matters after hours, that cannot wait until the next business day, you may call us to be connected to our answering service, who will ask you about the nature of the problem. If the problem is urgent and related directly to your care with OSC, the answering service will contact the physician on call.

Please understand that the doctor on call cannot take care of any prescriptions of any kind, and definitely cannot prescribe any pain medications.

We require a 48-hour notice on all prescription refills. Please call your pharmacy and have them fax a refill request to our office. We do not manage medication for chronic pain. Please keep in mind that all prescriptions must be handled during normal office hours: Monday through Friday, 9-5. Our on-call physician is unavailable for prescription refills.
